Home Health Infusion Nurse
Overview As a key member of the interdisciplinary care team, the Home Health Infusion Nurse coordinates the delivery of care and services appropriate to the needs of the patient and their families. This includes physical and psychosocial assessment, identification of interventions, care plan development, education and monitoring. This position must be able to work independently, be detail oriented and handle multiple tasks in a fast paced environment. Additionally, this position requires a reliable means to travel to multiple sites throughout the shift for patient visits.
