The role you’ll contribute:
GENERAL SUMMARY:
The Oncology Licensed Social Worker (OLSW) is a specialized professional who assists the oncology patient and family to identify social, emotional, and environmental needs, and to connect them with available resources/services through the provision of a full spectrum of social work services. The OLSW will focus on the following areas of service: patient/support partner(s) and communication with healthcare providers for identified needs, and referrals to identified needed community services.
PRINCIPAL DUTIES AND JOB RESPONSIBILITIES:
Promotes knowledge of oncology and effects for patient, support partner(s), and care providers
Meets with patient/support partner(s) after diagnosis to provide counseling and referral services specific to needs.
Collaborates with direct care staff to provide ongoing support for patient.
Assesses patient needs for care resources.
Collaborates with existing community organizations for patient needs.
Educates and collaborates with multidisciplinary team to plan and participate in quality initiatives and research projects as appropriate.
Reviews patient information and assesses patient for emotional needs and risk-factors.
Serves as customer service advocate for patient and support partner(s).
Consistently collaborates with physicians and multidisciplinary team to establish and oversee plan of care.
Reviews physician and nurse documentation to ensure patient needs are met.
Serves as primary point of contact for providers and patients requiring the OLSW. Acts as a resource to
Infusion/Oncology/Radiation Practices for patients' needs. Collaborates with patient to identify potential barriers to achieving oncology treatments and set health goals for quality of life.
Performs patient/family evaluations and histories. Provides psychosocial support through individual or family counseling, as needs dictate. Continuously reviews services area for needs and opportunities.
Demonstrates skills and knowledge necessary to provide care appropriate to age of patient and family, including the knowledge of growth and development, the ability to obtain and interpret information to identify patient needs, and to provide the care needed.
Collaborates with Infusion nursing to confirm continuity of care needed.
Works with Navigators to provide Survivorship Care Plans to patients.
Contributes to community programs and education pertaining to specific oncology services.
Maintains a working knowledge of the availability of community resources and financial services for providing support and reducing barriers to care.
Fosters supportive professional partnerships with community physicians.
Participates in select community events to promote specific oncology programs at AdventHealth Gordon.
Collaborates with community resources commonly used for oncology care needs.
Qualifications
KNOWLEDGE AND EXPERIENCE STRONGLY PREFERRED:
KNOWLEDGE AND SKILLS REQUIRED:
Able to communicate effectively in English, both verbally and in writing required.
KNOWLEDGE AND EXPERIENCE PREFERRED:
Additional languages preferred.
EDUCATION AND EXPERIENCE REQUIRED:
Master's Degree in Licensed Social Work required.
EDUCATION AND EXPERIENCE PREFERRED:
At least ¬3 years of recent social work experience in oncology preferred.
LICENSURE, CERTIFICATION OR REGISTRATION REQUIRED:
Current licensure in the State of Georgia for Social Work required.
LICENSURE, CERTIFICATION OR REGISTRATION PREFERRED:
Professional Certification in specialty area preferred (or within three years of hire)
