The Licensed Practical Nurse is responsible for observing, recording and reporting changes in the patient's condition, as well as assisting in the development and revision of the patient-driven holistic care plan for life. The LPN is a member of the larger team and reports to the primary care manager. The LPN requires a general review of their work by an RN for appropriateness and conformity with license, policies and procedures. Applicants pending the completion of educational or certification/licensure requirements may be referred and tentatively selected but may not be hired until all requirements are met.
