Clinical Nurse to join the team at their Corvallis, OR location. An integral part of our overall mission of creating a patient-centered experience, the Clinical Nurse will be the supportive team member that will provide support to current patients with questions about their care. Responsible for triaging patient calls and assessing for severity and complexity of the compliant, the Clinical Nurse helps provide care for patients in between visits.

Responsibilities:
- Prioritize and assess the needs of the patients in clinic and over the phone and to utilize resources, knowledge of the cancer disease state, and clinical judgement to determine the best solution for the patient's current issue
- Triage patients calling into the practice by following written protocols that include verifying patient info and status, gathering pertinent information, use critical thinking to determine severity of compliant, document interaction, and refer to ED if needed.
- Will use clinical assessment skills and best judgement to determine if emergency care is needed immediately, the issues should be addressed at urgent care, or if the complaint can wait till the patients' next visit
- Work from a queue of non-emergent messages within 24 hours
- Documents all conversations with patients to maintain a comprehensive medical record
- Provides clear instruction and education to patients via the telephone
- Collaborates with physicians via electronic communication, face to face, or the telephone to discuss patient care needs that cannot be independently and appropriately addressed using standing orders or practice protocols
- Sets up same day and/or future provider/infusion room appointments to address symptom management concerns
- Communicate in a respectful and therapeutic manner, offering compassion and understanding at every interaction
- Serve as a clinical resource and appropriately delegate, coordinate care and collaborate with others as equal members of the inter-professional care team to integrate nursing knowledge, model professional behaviors as a representative of the nursing profession
- Serve as a key communication link between the patient, his/her family and the care team
- Ensure accurate and timely documentation of all encounters, consultations and treatment are recorded in the patient's record via Electronic Health record system
- Utilize all resources and guidelines at his/her disposal to effectively assess, prioritize, advise, schedule appointments, or refer calls when necessary to the appropriate medical facility, personnel or specialized community service
- Maintain accurate, clear, and concise electronic documentation via notes, problem lists, and medication lists

- Demonstrates an ability to quickly prioritize and re-prioritize patient needs and tasks throughout the day
- Ability to navigate EMR and computer software systems
- Ability to navigate multiple computer programs, patient and hospital portals simultaneously
- Ability to present information in an organized framework-SBAR (Situation, Background, Assessment, Recommendation
Qualifications:
- Current RN state license with the applicable State Board of Nursing
- Minimum of 2+ years nursing experience, 1+ year of triage preferred
- Oncology or applicable specialty experience preferred.
- ONCC within 3 years of hire.
- Experience and knowledge of the purpose and use of medications in the treatment of cancer is preferred.
- Current CPR certification required.
